Five Wishes

The Tyndale Centre for Grief and Loss is pleased to offer its inaugural end-of-life planning workshop. Five Wishes is an established guided conversation that helps you identify your personal, emotional, spiritual, and medical preferences for the end of life. It assists you in selecting a trusted person to make healthcare decisions on your behalf if you are unable to do so. Additionally, it provides you with a written plan to communicate your wishes and treatment choices in the event of serious illness. This process helps promote essential conversations with your family before a crisis occurs.

Our session is designed to promote essential conversations that help you plan and communicate your wishes. Whether you're arranging your personal end-of-life plans, supporting a loved one through end-of-life care, or simply seeking general guidance, this event provides a proven approach to initiate this important dialogue.

$17 per person (Includes HST) 

After registering, you will receive a registration confirmation email, and your spot will be reserved. You will also be given access to a digital booklet. Please note: the Zoom link will be sent to the email address you registered with the day before the session (Friday, April 17).

Dr. David Sherbino

Professor of Spirituality and Pastoral Ministry

Professor Sherbino has been a faculty member at Tyndale Seminary since 1987, after serving as Chair of the Pastoral Ministries Department, Coordinator of the Worship and Liturgy program, and Dean of the Chapel. He also oversees the seminary's Pastoral Thanatology department, as well as its certificate, master's, and doctoral programs, and serves as the advisor to the Tyndale Spiritual Formation Centre and the Tyndale Centre for Grief and Loss. David is also a minister, psychotherapist, and certified thanatologist.
