Five Wishes Workshop

Five Wishes is an established, guided conversation that helps you identify your personal, emotional, spiritual, and medical preferences for the end of life. It also helps you choose a trusted person to make healthcare decisions on your behalf if you are unable to do so. Additionally, it provides you with a written plan to communicate your wishes and treatment choices in the event of serious illness. This process promotes essential conversations with your family before a crisis occurs.

This session is designed to foster essential conversations that help you plan and communicate your wishes. Whether you're arranging your end-of-life plans, supporting a loved one through end-of-life care, or simply seeking general guidance, this event offers a proven approach to initiate this important dialogue.

Presenter

Professor Sherbino has been a faculty member at Tyndale Seminary since 1987, after serving as Chair of the Pastoral Ministries Department, Coordinator of the Worship and Liturgy program, and Dean of the Chapel. He also oversees the seminary's Pastoral Thanatology department and its certificate, master's, and doctoral programs, and serves as advisor to the Tyndale Spiritual Formation Centre and the Tyndale Centre for Grief and Loss. David is also a minister, psychotherapist, and certified thanatologist.
